RESUMO

INTRODUCTION: This cross-sectional study assessed the association between sense of coherence (SoC) and caries activity (number of active caries lesions) and caries experience (DMFT index) among 18-19-year-old male adolescents who joined the Brazilian Army as draftees for mandatory military service (n = 507). METHODS: Data collection included a questionnaire (level of education, family income, and tooth brushing frequency), the Brazilian short version of the SoC scale (SOC-13), and clinical caries examination (noncavitated/cavitated, inactive/active). The main predictor variable was SoC, categorized as low, moderate, or high. Poisson regression models were used for statistical analysis. RESULTS: A high SoC was significantly associated with a lower number of active lesions (adjusted rate ratio = 0.85; 95% CI = 0.74-0.98). No association between SoC and DMFT was detected. CONCLUSION: A high SoC was found to be a protective factor to caries activity in this population.

RESUMO

This study aimed to compare the efficacy of 2.5% sodium hypochlorite (NaOCl), 2.5% calcium hypochlorite [Ca(OCl)2], and 2% chlorhexidine (CHX) in the rapid disinfection of gutta-percha cones contaminated with Candida albicans. The minimum inhibitory and minimum fungicidal concentrations of each solution for C. albicans were determined and the ability of each solution to destroy and inhibit biofilm in culture wells was tested. In addition, ninety-eight gutta-percha cones contaminated with the fungal suspension were disinfected according to the type of solution (2.5% NaOCl, 2.5% Ca(OCl)2 or 2% CHX) in its different application methods (without agitation, ultrasonic agitation or agitation with Easy Clean), and regarding the exposure time to each irrigating solution (1 or 5 min). Next, the samples were checked for turbidity and evaluation of viable colonies. The compounds that showed the best performance in biofilm destruction were NaOCl and Ca(OCl)2 at a concentration of 2xMIC (p < 0.001). Regarding inhibited biofilm, the only compound that was effective at all MIC concentrations tested was 2.5% Ca(OCl)2 (p < 0.0001). Regarding the viable colonies, all solutions were effective concerning the control group, for all application methods, in 1 and 5 min (p < 0.05). The densitometer reading showed that CHX was the only effective solution in all application methods performed (p < 0.05). The results demonstrate that all tested solutions were effective in the rapid decontamination of cones contaminated with C. albicans.

RESUMO

OBJECTIVE: The aim of this study was to systematically review observational studies assessing the association between socioeconomic status (SES) and traumatic dental injuries (TDI) in permanent dentition. METHODS: Electronic searches were performed in PubMed, EMBASE, Web of Science, LILACS, CINAHL, COCHANE Library and ScoINDEX databases for articles published up to February 2023. Two independent reviewers performed the search and critical appraisal of the studies. The inclusion criteria were observational studies that evaluated the association between individual-level socioeconomic indicators and TDI (clinically examined) in permanent teeth. Quality assessment of included articles was conducted using the Newcastle-Ottawa Scale. Global meta-analysis was performed with all studies and different subgroup analysis based on socioeconomic indicators (household income, educational level or any other indicator), age (children, early adolescents, late adolescents or young adults) and economic classification of the country (high, upper-middle or lower-middle). A random-effects model was used to estimate pooled prevalence ratios (PR) and respective 95% confidence intervals (CI) for each study. RESULTS: The search strategy retrieved 11 315 publications. According to eligibility criteria, 17 articles were included in the meta-analysis. Individuals with low SES were 17% more likely to have TDI (PR 1.17; 95% CI 1.05-1.30). The subgroup analysis also revealed that the indicator (household income, PR 1.16; 95% CI 1.00-1.34) and the economic classification of the country (upper-middle, PR 1.19; 95% CI 1.07-1.33) influenced the association of SES with TDI occurrence. CONCLUSIONS: Individuals with lower SES were more likely to present with TDI in permanent dentition than those with higher SES.

RESUMO

This study evaluated the association between sense of coherence (SoC) and oral health-related quality of life (OHRQoL) among conscripts of the Brazilian Army, in two cities of southern Brazil. A cross-sectional study included all 18-19-year-old adolescents who joined the Brazilian Army as draftees for mandatory military service in the cities of Itaqui, RS, and Santiago, RS (n = 505). Data collection was conducted from 2019 to 2021, and included the application of questionnaires and a clinical oral examination to record gingivitis, malocclusion, and dental caries. OHRQoL was collected through the Brazilian short version of the Oral Health Impact Profile (OHIP), composed of 14 questions. The adolescents' SoC was assessed using the validated Brazilian version of the SOC-13 scale. The primary outcome of this study was OHRQoL, modeled as a discrete variable (OHIP-14 scores). The main predictor variable was SoC, categorized as low, moderate, or high. The association between predictor variables and OHRQoL was assessed by Poisson regression models using a hierarchical approach. Unadjusted and adjusted rate ratios (RR), and 95% confidence intervals (CI) were estimated. All the analyses were performed using STATA software version 14.2. Adolescents with a moderate and high SoC had 27% (RR = 0.73, 95%CI = 0.64-0.84) and 51% (RR = 0.49, 95%CI = 0.41-0.58) lower mean OHIP-14 scores, respectively, than those with a low SoC score, after the inclusion of behavioral and clinical variables. This study showed a significant association between SoC and OHRQoL among 18-19-year-old southern Brazilian adolescents. Strengthening the SoC as a psychosocial resource may improve the well-being and OHRQoL of adolescents.

RESUMO

The aim of this prospective cohort study was to assess the radiographic progression of underlying dentin shadows (UDS) on the occlusal surfaces of permanent posterior teeth of adolescents and young adults over 1-2 years and to identify possible risk factors. A total of 149 UDS lesions (from 101 individuals) were included at baseline. Each participant had to present at least one UDS to be considered eligible for the study. Data collection included the application of a questionnaire, clinical examination, and bilateral bitewing radiographs, performed at baseline and after 1-2 years. The association between possible predictors and UDS progression (defined radiographically as an increase in the radiographic score from baseline to follow-up) was assessed using Weibull regression models. Hazard ratios (HRs) and their 95% confidence intervals (CIs) were estimated. A total of 81 individuals (mean age: 24.0, standard deviation: 8.03) were reexamined after 1-2 years (742 occlusal surfaces, of which 118 were UDS). The overall progression rate was 8.6% after 1-2 years, being 12.6% for UDS without baseline radiolucency and 20% for UDS with baseline radiolucency. The risk analysis showed that UDS without radiolucency at baseline had a similar likelihood of progression (adjusted HR = 1.71, 95% CI = 0.68-4.32, p = 0.26) while UDS with radiolucency at baseline were more likely to progress (adjusted HR = 2.96, 95% CI = 1.06-8.26, p = 0.04) than the reference category (sound occlusal surfaces without radiolucency). These estimates were adjusted for caries prevalence, tooth type, and arch. This study showed low progression rates of UDS after 1-2 years. The presence of radiolucency at baseline was found to predict UDS progression.

The case is about a girl in which a dental trauma resulted in a fracture of a maxillary central incisor with incomplete root formation, compromising both the esthetics and the vitality of the tooth. Endodontic treatment was performed with apexification and then apical MTA plug. The tooth was definitively restored with composite resin and the patient was followed up. In the follow- up consultations, there was no painful symptomatology and the patient was satisfied with her current appearance. Thus, the apexification presents itself as a great alternative for teeth with incomplete root formation and pulp necrosis, although in the long run there was an aesthetic improvement and patient satisfaction.

Pulp calcifications cause morphological changes inside the root canals that make endodontic treatment difficult. To solve this difficulty, the tool Endoguide or Guided Endodontics was developed to solve complex cases. This research aimed to describe the applications of the Endoguide tool in the treatment of calcified root canals in endodontics. All searches were performed by a single researcher in the PubMed/MEDLINE database and manual search using the descriptors "Endodontics", "Digital", "Cone beam", "Guided". Articles published between 2010 and 2022 were considered eligible, and studies whose publication was not obtained in full text were excluded, as well as those in which the results did not have sufficient theoretical and practical basis for them to be included in the study. After the searches, 47 articles were found, initially selected by title and abstract, excluding duplicates. In the end, the complete reading and data collection was performed with 6 articles. Relevant information from the selected articles was transcribed for a table to summarize the findings. Guided endodontics can be an excellent alternative for treating complex cases, requiring minimal financial investment in the office, as image capture equipment, virtual planning and guides can be performed in trained laboratories. Thus, we conclude that the use of Endoguide as a tool in the treatment of calcified canals proved to be successful and its use does not require extensive knowledge and can be used by less experienced professionals.

RESUMO

Abstract This study evaluated the association between sense of coherence (SoC) and oral health-related quality of life (OHRQoL) among conscripts of the Brazilian Army, in two cities of southern Brazil.

RESUMO

Background and Objective: This study aimed to assess the prevalence, extent, risk indicators, and intraoral distribution of underlying dentin shadows (UDS) in the occlusal surfaces of the permanent posterior teeth. Subjects and Methods: A total of 1197 adolescents were included in the study (participation rate of 72.3%). The association between the independent variables, on the one hand, and the prevalence and extent of UDS were assessed using multilevel Poisson regression models. The prevalence ratios (PR), rate ratios (RR), and 95% confidence intervals (95% CIs) were estimated. The overall prevalence of UDS was 8.8% (95% CI = 6.0-11.5), with an overall mean of 0.13 (95% CI = 0.08-0.17) lesions per individual. Those with UDS exhibited a mean of 1.45 (95% CI = 1.28-1.62) lesions. Results: Lower family income (PR = 1.58; 95% CI = 1.03-2.41) and caries activity (PR = 1.53; 95% CI = 1.01-2.31) were significantly associated with UDS prevalence, while age ≥17 years (RR = 1.43; 95% CI = 1.03-1.99) and caries activity (RR = 1.63; 95% CI = 1.55-2.30) were significantly associated with caries extent. The most commonly affected teeth were lower first molars, followed by upper first molars and lower second molars. Conclusion: This study found a low prevalence and extent of UDS in the occlusal surface. Sociodemographic variables and caries activity were associated with the occurrence of UDS.

RESUMO

This study aimed to assess the prevalence, extent, and associated factors of underlying dentin shadows (UDS) in the occlusal surfaces of permanent posterior teeth among 12-year-old schoolchildren from southern Brazil. A population-based cross-sectional study that included a representative sample of 1,528 schoolchildren was conducted in Porto Alegre, southern Brazil (participation rate: 83.2%). A questionnaire on sociodemographic characteristics, behavioral habits, and access to dental services was sent to parents/legal guardians of each student selected. Caries examination included the recording of non-cavitated and cavitated lesions, including the presence of UDS, as defined by the ICDAS Group (i.e., a shadow of discolored dentin visible through an apparently intact enamel surface that may or may not show signs of localized breakdown - ICDAS Code 4). The association between predictor variables and UDS prevalence was assessed using survey Poisson regression models. The prevalence of UDS was 6.3% (95%CI = 1.2 -11.3). The mean number of affected teeth among the schoolchildren presenting UDS was 1.51 (95%CI = 1.40 -1.61), ranging from 1 to 6. Type of school (public, PR = 2.23, 95%CI = 1.22 -4.07) and caries experience (DMFT 1-2, PR = 2.41, 95%CI = 1.15 -5.04; DMFT ≥ 3, PR = 3.09, 95%CI = 1.52 -6.27) were significantly associated with UDS prevalence. In conclusion, this population-based cross-sectional study found a low prevalence of UDS in the occlusal surfaces of the permanent posterior teeth of 12-year-old southern Brazilian schoolchildren. Overall caries experience and type of school were associated with UDS in this population.

Bacterial endocarditis (BE) is an infectious disease with several etiological factors, including the constant presence of oral bacteria. Given this relationship and the relevance of the subject, it is essential that dentists deepen their knowledge on this matter. Thus, the aim of the study is to conduct a literature review on the mechanism of BE development and its relationship with oral hygiene. Searches were conducted in the PubMed, EMBASE and Biblioteca Virtual em Saúde (BVS) platforms, using the descriptors previously selected from the MeSH and DeCS platforms. After analyzing their titles and abstracts and then read in full, nine articles were selected for the literature review. The findings show that poor oral hygiene in individuals with cardiovascular disorders may be a predisposing factor for the development of BE, due to the interaction of microorganisms and pro-inflammatory mediators. Therefore, the understanding that maintenance of oral hygiene is essential to reduce microbial accumulation and aggregation and, consequently, BE, highlights the importance of dentists knowing the subject to prevent complications and act preventively.

Cleaning of root canals is a crucial step for a successful endodontic treatment. For this reason, the search for effective irrigating solutions in endodontic therapy of deciduous teeth is frequent in research and it demonstrates the need to find substances with lower toxicity and greater efficiency. Thus, this study aims to analyze the different irrigating solutions and protocols in the endodontic treatment of deciduous teeth, in order to build a clinical approach. For this, a literature search was performed in the Databases PubMed/ MEDLINE, Cochrane Library, SCIELO, Portal de Periódicos CAPES and VHL using the combination of the terms "deciduous tooth," "root canal irrigants," "pulpectomy" and their derivatives, associated with each other by the Boolean operator "AND" and adapted for each database. The searches were carried out from July to August 2020. In total, 192 results were found. After a critical reading, eight studies were selected. The review included studies addressing the most common irrigating solutions: sodium hypochlorite, chlorhexidine digluconate, citric acid, and ethylenediaminetetraacetic acid (EDTA) in endodontic therapy of deciduous teeth. EDTA and chlorhexidine digluconate have low toxicity and the 1% sodium hypochlorite combined with 6% citric acid have the most prosperous combination. The association of these two substances proved to be more efficient in cleaning root canals and removing the smear layer. However, more studies are necessary, mainly clinical ones, to confirm the best irrigating protocol for the endodontic treatment of deciduous teeth.

Objective: to analyze information in the literature about mentioned pathology with a dental focus, based on current evidence in order to structure a clinical conduct, preventive and therapeutic methods. Literature Review: literature searches were performed in December 2020, using pre-established descriptors, in the databases LILACS and PubMed/MEDLINE, in addition to complementations using Google Scholar. About 563 articles were found, refined in 10, along with the use of 3 books. Final Considerations: From the findings, it was found that the performance of a careful anamnesis during the first consultation is essential, as it helps to understand the patient's pathophysiological aspects, which will determine the use of drugs, mainly, before invasive procedures. On the other hand, this matter was neglected in the Dental field, due to the fact that there are few studies related to the clinical management of patients with Rheumatic Fever (RF), and further research is necessary, with the aim of building an adequate clinical conduct, reducing the risks and the incidence of such a disease.(AU)

RESUMO

Abstract This study aimed to assess the prevalence, extent, and associated factors of underlying dentin shadows (UDS) in the occlusal surfaces of permanent posterior teeth among 12-year-old schoolchildren from southern Brazil.

Objective: to assess the predictability of edentulous maxillary rehabilitation using the all-on-four technique, measured by tensions over implants in laboratory studies and prosthetic survival, marginal bone loss and implant success in clinical studies. Literature Review: A literature search was performed in the Lilacs, SciELO and PubMed/ Medline databases, using terms related to the theme, without time restriction and adapted for each base. The searches were carried out by a single examiner from September to November 2019. After a critical reading of the 21 selected articles, it appears that there are several studies with many designs that evaluated the predictability measured in various ways. Final considerations: In view of the scientific evidence found, the rehabilitation of edentulous jaws with the all-on-four technique proves to be predictable and stable. Retrospective, prospective studies and systematic reviews report high success rates, low marginal bone loss, basing the use of the all-on-four technique in dentistry.(AU)

RESUMO

Religiosity has been associated with a better dental condition and positive self-perception of health, but there are no studies on the relationship between religious practice and oral health-related quality of life (OHRQoL), especially among schoolchildren. Thus, the aim of this study was to evaluate the association between family religiosity and OHRQoL in 12-year-old schoolchildren. We conducted a cross-sectional study in a representative sample of 12-year-old schoolchildren from Santa Maria, a city in southern Brazil. A total of 1,134 schoolchildren were assessed in a randomly selected in 20 public schools in the city. Participants were examined by 4 calibrated dentists (minimum Kappa-value for intra and inter-examiner agreement were 0.79 and 0.77, respectively) according to dental caries (Decayed, Missed, Filled Teeth Index), and gingival bleeding (Community Periodontal Index criteria). OHRQoL was assessed by the Brazilian short version of Child Perceptions for Questionnaire (CPQ11-14). Parents or guardians answered a structured questionnaire regarding their socioeconomic status and religious practice. Data were analyzed using multilevel Poisson regression analysis to assess the association of religiosity and overall and domain-specific CPQ 11-14 scores. Religious practice less than once a week was associated with higher mean symptoms domain scores and higher mean CPQ 11-14 overall scores in schoolchildren. In conclusion, our findings demonstrate that family religiosity was positively associated with schoolchildren's OHRQoL.
